Have you maybe considered modifying the "extension_dir" setting in php.ini (/etc/php5/apache2/) so that it points to /usr/lib/php5/20090626+lfs/? Was the IcePHP.so the only extension that was installed in 20090626+lfs?

I was trying to reproduce your issue, but with a fresh PHP5, Apache2 and zeroc-ice34 installation everything works OK on my Ubuntu 12.04.

I fully agree here. If you don't need Ice support for PHP, you could consider removing IcePHP.so manually, to get rid of the error message.
