[ome-users] Import dies with foreign key constrain violation

I'm importing TIFF images in OME 2.6.1
The import fails with the following error:
insert or update on table "actual_inputs" violates foreign key constrain "$3" detail: KEY
MODULE_EXECUTION_ID = 121 IS NOT PRESENT IN TABLE MODULE_EXECUTIONS

What happens? Is it possible to fix it?
